This Annual CHR Summit will: Provide skills and training to help support CHRs to expand professionalism and collaboration Recognize and celebrate positive impacts CHRs make in American Indian Communities Connect valuable information and resources regarding CHR Policy Development, Integration, and Sustainability Agenda: CHR Summit X Final Agenda Awards: The CHR Summit planning committee is accepting nominations for the following awards: AWARD NOMINATIONS ARE CLOSE D Pre-Conference Details: 📅 Monday, October 26, 2026 🕗 11:00 AM - 4:00 PM Session 1 12:00 PM - 1:15 PM (limited to 30 CHRs) Session 2 1:15 PM - 2:30 PM (limited to 30 CHRs) Session 3 2:30 PM - 3:45 PM (limited to 30 CHRs) Banner Health Plans' team of Registered Nurses (RNs), Certified Diabetes Care and Education Specialists (CDCES), and Registered Dietitians (RDs) will provide hands-on competency training for Community Health Representatives (CHRs). Participants will learn essential skills to support individuals living with diabetes and other chronic conditions, including proper use of a glucometer, application of a continuous glucose monitor (CGM), accurate blood pressure cuff placement and measurement, and diabetes foot checks. Through demonstration and practice, CHRs will gain practical skills to promote health education, early identification of concerns, and improved community health outcomes.
