Banner Health Plans' team of Registered Nurses (RNs), Certified Diabetes Care and Education Specialists (CDCES), and Registered Dietitians (RDs) will provide hands-on competency training for Community Health Representatives (CHRs). Participants will learn essential skills to support individuals living with diabetes and other chronic conditions, including proper use of a glucometer, application of a continuous glucose monitor (CGM), accurate blood pressure cuff placement and measurement, and diabetes foot checks. Through demonstration and practice, CHRs will gain practical skills to promote health education, early identification of concerns, and improved community health outcomes.
